A new health center is being established at Erasmusweg 137 in Hoogezand (the Netherlands) in the former school building of the Aletta Jacobs College; the Medical Center Hoogezand-Sappemeer. For years, initiators have worked hard to interest care providers, to obtain financing and to make agreements with the municipality about the purchase of the former school building. Ultimately, care is provided here to the patients of five general practices through a multidisciplinary integral collaboration in and from a joint building.
